What is a D'Viajeros form?

The D'Viajeros form (Cuba Traveller’s Form) is an online entry form all visitors must complete before arriving in Cuba. It collects your personal, travel, and health information and gives you a QR code to present at immigration.

Why do I need it?

Cuban authorities require the form for entry. It records passport, flight, accommodation, and health details, helping speed up immigration at airports in Havana, Varadero, Cayo Coco or Holguin.

How do I complete the form?

Complete the form online before departure. After submitting, you’ll get a QR code to print or save on your phone for immigration.

When should I submit it?

Submit the form within 72 hours (3 days) before your flight to ensure your information is current.

What information do I need?

You’ll need:

  • Passport details
  • Flight and travel dates
  • Accommodation in Cuba (hotel, resort, or casa particular)
  • Contact details
  • Health and customs info
  • Travel insurance details

Do all travellers need it?

Yes. All visitors, including those with a tourist card or e-visa, must complete the D'Viajeros form.

Can I change it after submitting?

If your travel plans change, you usually need to submit a new form with updated details.
